Claims Guide

By Class of Insurance

Motor Accident

  • Take details of the location and cause of the accident.
  • Notify us immediately. NEVER admit liability to third parties.
  • Take details of the third party and their insurers.
  • Obtain a repair estimate; don't authorise repairs without insurer consent.
  • Send us the completed claim form, original police abstract, logbook copy and driver's licence copy.
  • Obtain an inspection report in case of injuries.

Motor Windscreen Claim

  • Replace the damaged windscreen.
  • Send us the original replacement receipt with ETR, copy of logbook and driver's licence.

Domestic Package

  • Report to the police station.
  • Take immediate action to minimise the loss.
  • Notify us as soon as possible.
  • Send us the completed claim form and a repair estimate where applicable.

Group Personal Accident

  • Notify us of the injury immediately.
  • Submit the completed claim form, medical bills and 3 months of payslips prior to injury.

Burglary

  • Ascertain the amount lost and notify us quickly.
  • Make an official police report and forward us the abstract.
  • Send us the claim form, an inventory of stolen items and any repair quotation.

Fidelity Guarantee

  • Notify us immediately on suspicion of employee fraud or dishonesty.
  • Notify the police; withhold any monies due to the suspect.
  • Submit the claim form, reference letters, police abstract, termination letter, suspect ID and appointment letter.

Machinery Breakdown

  • Advise us immediately.
  • Send the completed claim form with a detailed estimate.
  • Obtain an Engineer's report.

Marine

  • Comply with the cargo inspection clause (CSU) of the Marine Policy.
  • Forward Bill of Lading, invoice(s), packing list, MPRO, consignment notes, survey reports, third-party correspondence and import entry.

Money

  • Report the loss to the nearest police station.
  • Ascertain the amount and advise us as soon as possible.
  • Send the claim form and supporting documents (e.g. 3 months of bank statements, sales invoices).

Fire & Consequential Loss

  • Notify us immediately. Don't start repairs without insurer authority.
  • Send us the claim form, repair/replacement estimates and a costing breakdown.

All Risks

  • Notify us immediately.
  • Send the claim form with purchase/replacement receipts and any relevant accessories.

Work Injury Benefits Act (WIBA)

  • Report fatal claims within 24 hrs and non-fatal within 7 days.
  • Complete Labour Form DOSH Part 1 and have DOSH Part II completed by the attending doctor.
  • Submit forms to Labour Office for assessment (WIBA 4) and return to us.
  • Provide written statements from claimant, eyewitness and supervisor.

Life Insurance

  • Report the matter to us immediately.
  • Submit the claim form with original death certificate, burial permit, deceased's ID copy and any other supporting documents.

Medical Insurance

  • Inpatient, outpatient, dental, optical, critical illness and group excess-of-loss cover available.
  • Submit the claim form with original receipts, doctor's prescription, lab requests and supporting documents.
